Our Community Resources Department Provides:

IN-HOME SERVICES

In-Home Services support persons served and their families to help them achieve their ultimate goal of living and participating in their communities. Services are provided in eight counties -- South Region: Clark, Crawford, Floyd & Harrison Counties. North Region: Lawrence, Orange, Scott & Washington Counties -- and include community- and residential-based habilitation, respite, personal services and attendant care.

In-Home Services are crucial to prevent institutionalization in disabled populations. Services are provided at home, work, and in other places within the individual’s community. Instructors work with consumers in a variety of ways, including personal care and safety, leisure and social activities, decision-making, and self-advocacy.

HABILITATION

Provided for persons who are in need of improving and acquiring developmental and life functioning skills, this program offers training activities to assist individuals in developing and improving independence. Personal needs, social and community awareness activities are taught in real-life situations. Through person-centered planning, the program focuses on the person’s needs, desires and strengths in all areas of their lives. By identifying an individual’s goals, an Individual Service Plan can emphasize skills needed to achieve them.

STRUCTURED FAMILY CAREGIVING

This program is designed to establish a support system for individuals with disabilities and to assist them with daily living skills. The program also provides residential foster care by locating responsible and caring families willing to provide services and programming to adults with disabilities. Individuals are placed in a family setting and integrated into the family’s home life setting. These programs are offered to clients on a 24-hour basis.

WELLNESS COORDINATION

This is the newest program added to Community Resources in June 2014. A registered nurse provides oversight of the individual’s healthcare, with weekly consultation and at minimal monthly visits. The program is designed to assist individuals with preventive healthcare solutions to minimize hospitalizations.

PERSONAL SERVICES AGENCY

Blue River is funded and certified by the Indiana State Board of Health as a Personal Services Agency. Personal services include attendant care, which assists people with disabilities with daily living activities. For additional information on funding, please contact the local Area Agency for Aging. The Personal Services Agency office is located at 405 North Capitol Avenue Suite 208, Corydon, IN.
